Job Title: Program Evaluator
Salary: $19.44 per hour

ESSENTIAL DUTIES
- Monitor and evaluate data collection and quality improvement initiatives throughout the grant.
- Design and develop data collection and program evaluation tools.
- Identify program improvement opportunities, working closely with program staff.
- Conduct reviews of program-related documents, data files, published information, and official reports.
- Conduct routine analysis on program process and outcome data.
- Develop logic models and strategic plans in collaboration with project staff and community coalition members.
- Establish networks and coalitions within the local community to achieve grant program/activity goals.
- Work with Native American worksites to review policies on breastfeeding to reduce post-partum depression and childhood diabetes.
- Provide ongoing feedback, technical assistance, and support to program staff in the evaluation and implementation of grant goals and objectives.
- Assist with the production of the mid-year, final, and re-application applications and reports.
- Review program activity results/findings to compare with national/state/local trends for change and improvement.
- Create and implement a needs assessment tool to collect data within the community.
- Compare desired and actual program outcomes using monthly/quarterly reports.
- Attends staff meetings
- Performs other duties as assigned.
Minimum Requirement
Bachelor’s degree in Public Health or Human Services related field with emphasis on research methods or statistics. One (1) to three (3) years of experience in chronic disease education, health coaching, and/or public health related field, preferably in a health care setting. Valid Arizona Driver’s License; Class 1 Fingerprint Clearance; First Aid & CPR; HIPAA certification. Must also pass a criminal background investigation.
Knowledge, Skills, and Abilities
- Understands how to create data collection tools and conduct research.
- Working knowledge of diabetes, heart disease, and obesity.
- Ability to multitask and provide chronic disease support and care in a culturally competent manner with adults and families.
- Strong skills in communication, preferably strong interpersonal skills as applied to interactions with staff, supervisor, patients, community, and other agencies, sufficient to exchange or convey information quickly, professionally, and correctly.
- Strong skills in writing. Able to prepare and maintain documentation, required reports, and the collection and analysis of data gathered for the purpose of submitting reports.
- High proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and ability to learn other software applications
