Case Manager (Reach Ur Life)
Salary: $40,000 annually

ESSENTIAL DUTIES
- Develop and maintain active two-way communication with program participants, project personnel, and outside agencies regarding Reach Ur Life services.
- Facilitate completion of a timely, well integrated, and comprehensive Case Management plan for youth/adolescents including eligibility determination, initial intake, planning, coordinating, and scheduling.
- Coordinates directly with treatment providers, including Behavioral Health to track and ensure follow services are being provided. Facilitates transfer as necessary.
- Collaborates with internal and external agencies to promote, develop, and implement Zero Suicide initiatives.
- Produces Case Management reports for department personnel
- Collaborates with Project staff and schools identified to participate in the Reach Ur Life project.
- Tracks the progress of referrals and outcomes of the school prevention training and coordination.
- Assist and coordinate training and conferences as needed with appropriate personnel.
- Assist in collection of program data related to Early, Identification, Referral and Follow up performance measures.
- Develops effective working relations and cooperates with various schools and youth-serving organizations.
- Advocates for youth and adolescents as well as the Reach UR Life Program’s work in the community and within the organization.
- Attend weekly department staffing meetings and interagency meetings as required.
- Acts as a liaison between youth and NACA as well as external service providers and community partners.
- Performs other duties as assigned.
Minimum Requirement
Bachelor’s degree in social or behavioral science or related field and two (2) years of previous experience in social services. Experience in working with Native Americans is desirable. Preferred: Experience with suicide and loss, including bereavement counseling. Experience with mental health and related services system. Level I Fingerprint Clearance, CPR, First Aid, and HIPAA Certification. Possess a valid Arizona Driver’s License. Must pass a criminal background investigation.
Knowledge, Skills, and Abilities
- Knowledge of case management and care principles, practices, and procedures in order to assess patient care for relevant indicators to be used in the ongoing monitoring of client care.
- Knowledge of children’s mental health and related service system; demonstrated experience evaluation, policy analysis and strategic thinking.
- Excellent communication skills with the ability to work collaboratively with other health professionals.
- Familiarity with Native American culture(s), society and working knowledge and respect for the Native American culture, traditions and customs.
- Attend weekly department staffing meetings and interagency meetings as required.
- Ability to maintain strict confidentiality of client information and records.
