ESSENTIAL DUTIES

• Practices within the scope of the Arizona State Board of Nursing and maintains clinical competency, as demonstrated in providing and directing patient care.
• Adheres to industry standard of practice in the delivery of primary health care services.
• Coordinates assessment, treatment and management of children and adults through consultation with the supervising physician and other healthcare team providers to facilitate comprehensive patient care plans.
• Conducts labs, referrals and appropriate assessments of patients including history taking and physical examination consistent with scope of practice department protocol.
• Diagnoses or treats acute health care problems, illnesses, infections, and injuries.
• Analyzes and interprets patients’ histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
• Provide health care screenings for both preventative health and chronic conditions.
• Counsels and educates patients regarding reproductive health, preventative health, chronic conditions, disease prevention and pediatric health.
• Prescribes appropriate medications (dosage and route) utilizing the best practices and under the direction of the staff physician.
• Participates in performance measurement standards and tracking including, but not limited to, GPRA, UDS and ADHS requirements.
• Documents physical findings, formulates plan/prognosis based on patient’s condition and follow-up with patient in an accurate, timely way.
• Establishes and maintains communication with other health care team providers and departments (i.e. Health Promotion and Behavioral Health) to ensure operational efficiency and effectiveness and to promote consistency and continuity of care.

MINIMUM REQUIREMENT

Master’s Degree in Nursing from an accredited institution. Two (2) to four (4) years’ experience as a Family Nurse Practitioner in a clinical setting. Experience in working with Native Americans is desirable.  A current State of Arizona Advanced Nursing License as a Nurse Practitioner. Prescribing and dispensing privileges in the State of Arizona. This position is covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes. Must be current with required immunization per the NACA HR Policies and Procedures and Infection Control Policy. BLS Certification and HIPAA Certification. Possess a valid state Driver’s License. Level I Fingerprint Clearance Card. Must pass a criminal background check investigation in accordance with NACA Policy.

KNOWLEDGE, SKILLS, AND ABILITIES

• Knowledge of medical terminology. Knowledge of disease prevention, health education, and treatment for acute and chronic health issues.
• Ability to develop treatment plans for acute and chronic diseases.
• Ability to perform job using the principles and concepts of the patient-centered medical home model, evidenced-based practices, and shared governance in a multidisciplinary team model.
