ESSENTIAL DUTIES
- Assesses patients’ health status through utilization of the nursing process, physical examination, history taking, screening tests, and observation. Provides a complete, thorough, and written assessment of the patient on arrival utilizing the nursing
- Continuously collects data about the healthcare status of the patient/family in a systematic way to determine the need and type of care and treatment to be provided and the need for further assessments, using the iCare program through NextGen EHR.
- Informs the medical practitioner of changes in the patient’s condition, medications, and diagnostic results. Maintains statistical records and submits reports as requested. Keeps appropriate records of statistical data required by the agency for reporting
- Coordinates immunizations for the Family Health Center – orders vaccine supplies and maintains inventory control including management via the Arizona Vaccines for Children (VFC) Program. Responsible for reporting immunization usage to Arizona state, Coconino County, and Federal agencies.
- Provides case management and care coordination for high-risk patients. This involves coordinating appointments and referrals, obtaining prior authorizations, faxing necessary data to specialist referred to, and assisting with transportation
- Orders medical and laboratory supplies, maintains inventory control, and ensures all exam rooms are properly stocked with needed
- Reviews charts of all no-show patients to determine appropriate follow-up care. Tracks referrals made by medical practitioner to other facilities, specialists, etc. to ensure patient receives adequate
- Provides education to patient and patient’s support system in areas such as disease management and health prevention.
- Oversees management of appropriate NextGen packages (i.e. laboratory, immunization, women’s health).
- Collects data needed for GPRA, IHS Diabetes Care and Outcomes Audit and other required reports in accordance with grant and contract requirements.
- Perform other duties as assigned.
MINIMUM REQUIREMENTS
Associates Degree or Bachelor of Science Degree in Nursing. Two (2) years of nursing experience in an acute care clinic, community health, and/or hospital environment. Or equivalent combination of education and experience. Level 1 Fingerprint Clearance card, BLS, and HIPAA Certification. Possess a valid Arizona Driver’s License. Must complete/pass background check in accordance with NACA policy. This position is covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes. Must be current with required immunization per the NACA HR Policies and Procedures and Infection Control Policy.

- Knowledge of public health nursing practice, pediatrics, women’s health, and d
- Ability to perform nursing care procedures and techniques pertinent to ambulatory outpatient
- Ability to make independent judgments and work
- Ability to communicate with patients and staff of different economic, cultural, ethnic, and educational backgrounds.
- Demonstrates strong oral and written communication skills.
