Community Health Representative

Salary: $19.27 per hour

Job Type: Full-time (Non-Exempt)

ESSENTIAL DUTIES

  • Conduct community outreach and wellness activities focused on chronic disease prevention and management for diabetes, hypertension, heart disease, and obesity.
  • Provide patient navigation support, assisting patients and families in accessing community resources, clinical services, and culturally relevant programs.
  • Deliver health education using evidence‑based, culturally informed materials consistent with IHS, UIO, FQHC, and CDC guidelines.
  • Maintain strong communication and collaborative relationships with medical providers, behavioral health staff, and wellness program staff.
  • Access and document vital signs and relevant clinical information in the EHR as permitted by CHR scope; may view diagnostic results for care coordination purposes but does not perform or interpret diagnostics.
  • Meet all grant program goals and work plan objectives, including participation in health promotion initiatives such as physical activity programs, community advocacy events, cultural wellness activities, food demonstrations, educational workshops, and youth programming.
  • Collaborate with the Health Promotion team to deliver non‑clinical program components (approximately 50% of workload).
  • Provide coverage as assigned at wellness centers and community events.
  • Participate in the organizational Quality Improvement Program through incident reporting and identifying opportunities for improvement.
  • Perform other duties as assigned.

Minimum Requirement

High school diploma or GED. One-year experience in a clinical or clinical health setting.  Community Outreach/Chronic Disease (heart disease, obesity, and diabetes) Management experience preferred. Arizona Department of Health CHR license or work towards licensure to obtain within 12 months of employment as a condition of continued training compliance. This position is covered by Public Law 101-630, requiring contact or control over Indian children. Due to this requirement, the agency must ensure that persons hired for these positions have not been found guilty of or pleaded nolo contendere or guilty to certain crimes. Must be current with required immunization per the NACA HR Policies and Procedures and Infection Control Policy. BLS and HIPAA Certification. Must pass Level I Fingerprint Clearance and criminal background investigation.  Possess a valid State Driver’s License.

Knowledge, Skills, and Abilities

  • Knowledge of chronic health conditions and evidence‑based prevention and self‑management strategies.
  • Experience working with Native American/Alaska Natives.
  • Knowledge of medical terminology.
  • Ability to provide culturally grounded, trauma‑informed support to individuals and families.
  • Proficiency with technology, including Microsoft Office, NextGen EHR, Google Suite, Canva, and digital survey tools.
  • Ability to deliver group presentations and public health education in community settings.
  • Ability to maintain strict confidentiality in compliance with HIPAA.
